October is “Patient-Centered Care Awareness” month – a time set aside to remind us all that healthcare professionals are part of a healthcare delivery team that includes the patients and families we serve. It is vital that we all work together to ensure we are improving quality of life and outcomes.

Here at LHC Group, patient-centered care is the driving force behind everything we do. It’s a fundamental standard embedded deep within our company culture and represented in our founding mission statement – it’s all about helping people.

In recognition of Patient-Centered Care Awareness month, we’re highlighting our team members at Mississippi HomeCare of Flowood. Over the past month, the agency has received an overwhelming number of positive reviews on Facebook (no small achievement).

We wanted to hear – in their own words – how these outstanding home health providers view their roles as part of this team, and continue to impress the people they serve with quality patient-centered care on such a remarkably consistent basis.

Christy Bates, RN

“We treat our patients as family. For me, the most rewarding part is helping patients find solutions to their healthcare issues and keeping them in their home, out of the hospital, and as independent as possible. It’s important to treat all patients equally – as if they were your own family. We treat each patient as a whole, including their family in every step of the process, focusing on continuity of care, and following up with team members.”

From a patient review:
“My nurse, Christy Bates, was very kind and professional. She didn’t only treat me as a patient, she treated me like family.”

Jonathan Rinehart, Clinical Account Executive

“I believe great patient-centered care is defined as care and compassion toward the patient and their family. These two things greatly influence the kind of care provided. I am so thankful that our team practices these attributes day in and day out to provide the best possible experience for our patients.

“It’s all about doing the right thing! In delivering healthcare, there are far too many instances when patients suffer only because the healthcare worker does not do their job correctly and cuts corners. In this profession, we have a duty to our patient to always do what is right for them and provide exceptional care.”

From a patient review:
“Even during the week-long freeze we went through, the office would call me to check on me and make sure I had enough medical supplies – and even asked if I had enough food to carry me through any such storm.”

Jennifer Salter, RN, BSN, Care Transition Coordinator

“I think great patient-centered care comes from those who have a true passion for what they do – they want to care for patients in the very best way possible, every single time. They go above and beyond minimum expectations and deliver care with compassion and integrity.

“I visit and talk with patients while they’re in the hospital and coordinate their transition to home health. I remind myself that these patients aren’t there because they want to be; they are there because something has happened. Many times, that “something” is life-changing.

“This agency staff is an amazing team of clinicians. They regularly and willingly go above and beyond what’s expected in order to provide the best care. They truly love their patients – and their patients can feel that. The greatest reward is the joy and satisfaction of knowing that we’ve been even just a small part of a patient’s journey back to health and independence.”

From a patient review:
“Words can’t say how wonderful this company and nurses are. I’ve seen first-hand how this company stands out above the rest.”

Lauren Watkins, RN, Clinical Director

“Great patient-centered care means providing a supportive environment and individualized plan of care, ensuring that the patient and caregiver have the care and help they deserve. We care for our patients as if they are our own family. We make sure their anxiety with a new diagnosis or an upcoming procedure is decreased. With increased awareness of the disease process, they know what to expect – we are teachers as well as clinicians.

“We invest ourselves to the fullest in patient care, and we genuinely care about their progress. It’s a team effort to make sure we address all aspects of patient care. With this approach, we have been very successful. Having the ability to serve others is a blessing in itself. And being able to see progress with the patient and know you made a difference in someone’s life – nothing is more rewarding.”

From a patient review:
“The therapists were professional and concerned. They gave my husband a plan to do on his own that helps him gain strength and breathe better. We feel blessed to have Mississippi HomeCare of Flowood!”

Congrats to our team members at Mississippi HomeCare of Flowood – your performance is an inspiration to your LHC Group family!
